Muskegon County H1N1 Vaccination Information
Orchard View High School Vaccination Site
Dear Parent(s)/Guardian(s):
The Muskegon County Health Department has identified Orchard View High School as an H1N1 Vaccination Site. A one (1) day clinic will be held sometime between Monday, November 9 and Thanksgiving break. H1N1 shots will be made available to all OV high school and middle school students free of charge.
IMPORTANT: On Wednesday, November 4, 2009 high school and middle school students received a copy of an H1N1consent form to be signed by a parent/guardian.
Please complete the Muskegon County 2009 H1N1 flu vaccine consent form and return it to the High School or Middle School office NO LATER THAN NOON ON FRIDAY, NOVEMBER 6, 2009. This consent form reserves your son’s/daughter’s vaccination. Only students with a signed consent form on file will be vaccinated. There will be no make up day. Per the Health Department, forms turned in after this date will not be accepted.
A consent form may be picked up at the High School or Middle School office during business hours or downloaded HERE.
